In this condensed end-of-life care planning resource, you can record information that will be helpful to the loved ones you leave behind. Once completed, store it in a safe place so it will be available for use by your loved ones, and review it periodically to make changes as they arise.

Most pet owners worry about what will happen to their pets if something should happen to them. You can take control and make a detailed plan for your pet’s future. If you have multiple pets, be sure to complete one plan for each pet.
State of Oklahoma Advance Directive for Health Care.pdf
An Advance Directive for Health Care is a written legal document that allows you to instruct your attending physician whether or not you wish to be given life-sustaining treatments and artificially administered nutrition (food) and hydration (water) and to give other medical directions that impact the end of life. Its purpose is to recognize your right to control some aspects of your medical care and treatment, primarily the right to decline medical treatment or direct that it be withdrawn even if death ensues. An Advance Directive for Health Care may include a living will, the appointment of a health care proxy (a proxy is a person authorized to act for another) and directions for organ donation.
